Appointments
With the exception of mammograms, diagnostic imaging must be ordered by your physician. Your physician's office staff will help you to schedule your appointment. Mammograms may be scheduled directly by the patient.
It is extremely important for you to keep your scheduled appointment. If an emergency prevents you from keeping it, please call our office prior to your scheduled appointment time, to cancel or reschedule. This simple step will prevent unnecessary delays for all of our patients.
Documents to Bring
Please arrive thirty minutes before your scheduled appointment to allow yourself time to complete the necessary paperwork. Please bring the following documents to your appointment:
- Requisition Form - this is the form completed by your physician ordering this exam for you.
- Insurance Cards - you will need current insurance cards for whomever is paying the bill (self, spouse, or parent).
- Contact Information - please bring the name and telephone number of a friend or relative who does not live with you.
- Payment - a payment is required at the time services are rendered if you do not have insurance. We accept personal check, cash, money order, Visa, Discover or MasterCard. Discounts are available if services are paid in full at the time of service or within a short time after receiving your first statement. Financial assistance, based on need, may also be available.
Insurance
For our patients' convenience we are happy to file insurance claims. In order to do this you should bring your insurance cards to your appointment. Although we will file your claim, we cannot guarantee how your insurance company will reimburse for our services.
Contact your health insurance plan provider if you have questions about your coverage. Health insurance contact information is typically provided on the back of your insurance card.
